The army should give you a checklist, too, listing all the places you'll need to get signed off on to clear post/the army. Talk to someone at Army Community Services (that's where they do ACAP here at Ft Campbell, whoever is in charge of ACAP can help) or else your chain of command should be able to help.

Can't speak for the Army, but in the Air Force you're given an outprocessing checklist that you have to get signed off by various base agencies to ensure that you've done everything you need to do before you separate. Also, someone in Personnel gives you a briefing about what benefits you're entitled to (and you'll be entitled to a bunch since you're getting an honorable discharge) and how to go about contacting the Veterans' Administration so you can avail yourself of your VA benefits.

make sure you have medical follow-up lined up through VA hospital or your own doctor. You will need counselling for this readjustment period. Good luck.
