rheumatic disease is due to any one of these 1) bacterial infection by the Gram positive group A streptococcus 2) toxic effect of products of the grp A strept bacteria and 3)altered immune response to the (immunologically active) products of the grp A strept bacteria. Diagnosis is clinical, using " 'Jones Criteria' for Rheumatic Fever". It has Major and Minor Criteriae.
Updated Jones Criteria (1992 special writing group of the american heart association)
Major criteria
1. carditis 2.Migratory polyarthritis 3.Sydenham's chorea 4.subcutaneous nodules 5.erythema marginatum
Minor criteria (divided into Clinical & Laboratory)
A:Clinical --- 1. Fever 2. Arthalgia
B:Laboratory-----1.Elevated acute phase reactants 2. prolonged pr interval ( ECG finding)
PLUS
supportive evidence of grp A Streptococcal infection such as elevating streptococcal antibody levels // culture of grp A Streptococci from throat swab // positive rapid antigen detection
useful drugs(to be used under supervision of a physician) are penicillin, erythromycin, salicylates, prednisone

111111111Arthritis :
Rheumatic :- migratory, involves major joints(e.g. knee), asymmetrical, rarely involves small joints of hands / wrists/ feet/.
spares hip joints.
Rhematoid :- non-migratory---static, involves small joints(e.g. palm-finger[MCP] joints, joints of the wrist / feet), symmetrical (W.R.T. both sides of the body); MORNING STIFFNESS.

Rheumatoid: Immune-mediated

Rheumatic: rare; seen in long standing patients(chronic disease) subcutaneous -- extensor surfaces,
Rheumatoid: periarticular structures/ areas of mechanical pressure (extensor surfaces)/ pleura/meninges -- due to vasculitis ---- "Z" deformity of finger; "Swan Neck " deformity of finger; Boutonniere deformity; loss of PINCH

Rheumatic: (pro)long course of antibiotics, treatment of complications -- Heart vavle replacement // Valvuloplasty etc.
Rheumatoid: Aim: relief of pain / inflammation. Drugs are
NSAIDS (non-steroidal anti-inflammatory drugs),
DMARD(disease modifying anti-rheumatic drugs e.g. Gold),
steroids,
Immunosupression
TNF-alfa- Neutralising agents

I answered this in my medical final year exam
Luckily i had prepared a table differentiating these two
-------------------------------------------------------------------------
well prepared is half done, and totally incomplete
well prepared and a good start is almost done.
---------------------------------------------------------------------------
osteoarthritis is different than these two, and a third entity

Rheumatology, a subspecialty of internal medicine, is devoted to the diagnosis and treatment of rheumatic diseases. The term originates from the Greek rheuma, meaning "that which flows as a river or stream" and the suffix -ology, meaning "the study of". Rheumatologists mainly deal with problems involving the muscles and/or joints.
Rheumatism or Rheumatic disorder is a non-specific term for medical problems affecting the heart, bones, joints, kidney, skin and lung. The study of, and therapeutic interventions in, such disorders is called rheumatology. The term "rheumatism" is still used in colloquial speech and historical contexts, but is no longer frequently used in medical or technical literature; it would be fair to say that there is no longer any recognized disorder simply called "rheumatism". The traditional term covers such a range of different problems that to ascribe symptoms to "rheumatism" is not to say very much: arthritis and rheumatism between them cover at least 200 different conditions.A vast number of traditional herbal remedies were recommended for "rheumatism". Modern medicine, both conventional and complementary, recognises that the different rheumatic disorders have different causes (and several of them have multiple causes) and require different kinds of treatment. Most sources dealing with rheumatism tend to focus on arthritis. However "non-articular rheumatism", also known as "regional pain syndrome" or "soft tissue rheumatism" can cause just as much discomfort and difficulty.

Acute rheumatic arthritis: Another name for Acute rheumatic fever (or close medical condition association).
Rheumatoid arthritis
--------------------------------------------------------------------------------
Definition
Rheumatoid arthritis (RA) is a chronic autoimmune disease that causes inflammation and deformity of the joints. Other problems throughout the body (systemic problems) may also develop, including inflammation of blood vessels (vasculitis), the development of bumps (called rheumatoid nodules) in various parts of the body, lung disease, blood disorders, and weakening of the bones (osteoporosis).

Imagine for a moment your arthritis completely vanishing right now.
If, in a moment, your pain and stiffness melted away. And your joints all of a sudden functioned like a well-oiled machine.
Like a magic wand was waived, you'd be able to jump up from your chair and run around the room like a kid.
And imagine that you'd never have to suffer as much as a pinch of arthritis stiffness or pain ever again.
I know this sounds impossible right now, but bear with me, because sometimes miracles happen.
It was a gift from a woman on the other side of the world. From the kindness of her heart, she took pity on my suffering and taught me how to completely reverse my arthritis.
Today, I want to do the same for you!
