In addition to a passport, minors traveling outside the US need a "permission to travel form" signed by the parents (legal guardians) and notarized. A medical release form is also important. See the website below for a typical form.

It depends where you are travelling from, as your child may also need a visa.
If they don't require a visa then all you will need is a passport. The easiest thing to do to confirm this is to ask the travel agent!

You can ask your travel agent to give you a letter that says you give the granparents permission to travel with them out of the country...Just in case they ask, sometimes they do thats what my mom did when i was younger so i could travel with my dad. Even though he was my dad and my parents were married.

he will probably need a notarised letter from you stating that she has your permission to take him out of the country. I had so much trouble trouble travellingwith my children, because there is no father on their birth cirtificate, and i didn't have note

He might want to take his personal belongings you know cloths, food, toothbrush, things like that. but thats just my suggestion
