Say that we've been going out for a while and I really like you and I was wondering if maybe you would like to make it a little more exclusive by becoming my girlfriend.

Tell her your feelings for her have grown and no one else really matters anymore and from now on if she has the same feelings; you only want to see her. Either way it goes your relationship will not be the same after this.

"Can we have a dinner tonight?"Just treat her (never to a fastfood) in a fancy restaurant (don't be expensive)Bring something for her, a single rose,or a card or a small gift!Be gentleman, open a car door for her, drag the chair out for her and accompany her outside the restaurant powder room! And you must be the one to pay your bill!Don't ask her to be your GF on the first date,Just try to get her interest, ask her about herself and her family works or study, then after that ask her if she will go out with you again!

sit her down and ask her if she is ready to take it another level then there is no rejection and you will know how the girl feels
