Both...But more importantly, is That God will look at the state of our hearts come judgement day, and that is only possible by looking at what state our hearts are in at time of death.
For example, the right hand thieft lived a muderous and clearly sinful life, but gave a true deap heart-felt repentance in his final dieing seconds and Christ accepted him. Where as Judas live with Christ and was being preped up to be an apostle, and rejected christ and betrayed Him in the very last dieing seconds of his life and died in his own sin, thus he will be judged on that too.
But an even more interesting question is...when is our last dieing second? Can I guarantee that I will be alove this time tomorrow? Thus it is best to be prepared now rather than hope for the best for later.

Is there a last judgment? This is a specifically Christian teaching. I would hope that a person would strive to be good and possess a positive, moral code throughout his/her life rather than be selfish and make a last minute decision. That's like "having your cake and eating it, too."
I don't know if you mean the last, few years of a person or the last few minutes. People can change and grow, but I would be highly suspect of a deathbed confession.

,If you are a Christan, judgment is based on whether you have accepted Jesus Christ as your Lord and savior, and you understand and believe that you can be forgiven for all your sins through him. You also must remember to repent daily. In other words no, judgment is not based on your past, because when you accept Jesus you are completely new or born again.

I assume you're talking about whether or not you get into Heaven, in which case the answer is "neither." It's based on whether or not you have accepted Christ as your savior.
You will be held to account for how you spent your life, but the Bible is pretty vague on how that will work.

I think who you become now til the end because if you where bad all your life and stated a new life as of now and repent then it makes a big difference,as long as it comes from the heart.

I feel that everyone can change. So I would say that it would be based on who you truly are down deep. You can't just say that last week I killed someone but today I am a good person you have to truly be that good person.
