I eat adventurously and will try anything once, everything I've tried I would try twice, except this: dim sum restaurant had boiled chicken feet. It was like eating chicken fat with toe nails in it. I had to spit it out and do everything I could to avoid gagging

A tuna melt. I wanted no mayo, no butter, and as little tuna as they would put on it. Instead they smothered butter on both sides of the bread, put a whole can of dripping-with-mayo tuna in the sandwich and stuck disgusting super-processed cheese on top. Not to mention they FRIED the thing. I took out almost everything, did my best with the bread, and managed to down 1/4 of it before nearly puking. My grandparents wouldn't have liked it if I didn't eat it, and I didn't want to be rude. Arg, I can't beleive people eat these grease-smothered heart attacks on bread.

Oh yeah, and I'm a health freak in case you didn't notice.;)

Well it's not at a restaurant, but TJ's pizza (in aurora, colorado) -delivered has to be the worst pizza shaped pile of crap I even had the misfortune of ordering. It looked like Totino's and tasted like cardboard with ketchup wiped on. ---- YUCK

It was at Ruby foos in new york city...
I ordered a very simple noodle dish (because I thought that even one of these asian fusion chefs couldnt botch that). It came out, I ate, I spit into napkin and I got a refund. This dish had so much cilantro and basil in it that I almost vomited.

dressing with gravy. they used buttermilk in the gravy and it gave the dressing a very sour taste. either they don't know how to make the recipe tasteful using buttermilk or the buttermilk was bad!
