Blue Cross/Blue Shield is pretty good and not that costly compared to other providers.

Blue Cross Blue Shield or Aetna. Both have plans which have a low cost, but a high deductible. Kinda like catastrophic insurance. I chose one with a $5000 deductible, and it still covers preventative stuff (pap/mammogram) 100% and doctor visits are $25 copay and this includes prescription co-pays like $15/$25. The risk is if something goes terribly wrong, you pay the first $5,000.

blue cross blue shield isn't actually insurance, but it serves the same purpose.
As for the insurance, it really depends on the state because each state has its own laws. If you're in Alabama in the Birmingham area, you can call me at 205-655-4626 at Cahaba Insurance Agency and we can run competitive quotes of our companies we represent over the phone.
If you aren't in AL, I strongly reccommend an INDEPENDENT insurance agent that represents more than one company because they will be able to view the prices and coverages of multiple companies and will want to give you the best price because they get the same out of it either way.

All of the major health insurance companies now have "bargain basement" skeleton type plans. Call them and they can explain these plans to you. They are bare minimum plans. You get what you pay for.

I have Blue Cross Prudent buyer with a $2,500 deductible. It's the best I could find and still be affordable.

If you find an independent agent in your area, they can quote several providers. You can also adjust the monthly premium by choosing a higher deductible.
