The leucocyte (white blood cell, or esterase) test, if positive has the highest predicive value for UTI of all the tests on the dipstick. The nitrite test is second most important, but might be negative in a bonafide UTI due to enterococcus or Staph. Red blood cell or hemoglobin tests are often negative in bacterial UTI, but may be very strongly positive in viral cystitis or nephritis. The absolute, however, is the urine culture- the recovery of over 100,000 organisms per ml in pure culture of a clean voided urine specimen is the curent gold standard in diagnosis.

The don't test for infection, they tend to test for things that could indicate infection - like nitrites and white blood cells - as well as testing for sugar or protein. If a UTI is suspected they usually do a urine culture - they have you give a sterile urine specimen, where you wipe first and then pee in a previously sealed cup and then close it up again, and then they do cultures on it to find out if an infection is present. They can't do cultures on one of those "regular" samples we leave in the bathroom in a cup because it is not in a sterile container, amongst other things. I am actually waiting for results from a urine culture my doc had me give on Thursday. It takes a few days for the cultures to "grow" in the lab to see if any bacteria are present.

there are alot of things that can come back after a urine dipstick. white blood cells, red blood cells, nitrites, leukocytes (another type of white blood cell), protein...you dont want any of these things in there. bacteria is especially bad since the bladder is naturally a sterile environment. bacteria can be grown from your urine if it is collected and then sent for a culture, where bacteria is allowed to grow for an amount of time, and we can see what specific bacterias are there. this test takes ome time though. from there, a test can be run, called a sensitivity, to tell which antibiotics are best for the type of bacterias that are growing.

WBC(White Blodd Cells)- increased of this suggests infection (UTI), presence of bacteria (TMTC) Too Many Too Count...but the hallmark is subjective feeling of heat or mildly pain when urinating but some are asymptomatic
