Encourage everyone to enjoy and feel like kids again!
Do fun games, not too physically demanding though, that can get everyone laughing. A good mix of adult and older child games, some that use paper/pencil and some that are more interactive, would be great.
As far as favors or prizes, candies, nick nacks from the dollar store, candles, wind chimes, or useful items are always good.

For a theme, maybe "I didn't know that", even though they are family, I bet there are things that you don't know about each other, what a wonderful way of discover new and interesting things about each other. Maybe when they first arrive have each person, young and old write down something about themselves that others might not know, like a hobby or a favorite cartoon, or most embarrassing moment. Then at some point, someone else reads the slips of papers and everyone could guess who they think it might be. Another interesting group activity is those survivor challenges. Should be able to find them on the web. What it is, you divide your group up as castaway groups, mixing ages and experiences up. Then the challenge provides a list of various items that one might want to have with them if stranded somewhere, Each group decides on the ranking of items preferred to have and their reasons behind it. The challenge usually has a correct order of the list, and the real reason they should pick it. Then each group presents their decisions, man sometimes the logic used is, well let's just call it different and amusing. I think each group could even name themselves to make them more cohesive as a group. Another theme at this time of the year could be Harvest time, where everyone brings dishes made from fresh fruit or vegetables. Don't forget to make dirt cake for the kids, for some bizarre reason, they love that stuff, yuck! lol And if you have music in the background, remember to mix it up for all ages. Grandma and Grandpa can jump up and show them kids how the twist goes and the grandkids can teach them whatever it is that kids do now. I so miss family reunions, may yours be most enjoyable.

Games - bingo, some might like cards or Trivial Pursuit to keep the mind active.
Favors - I'd just suggest checking out your local dollar store. Also depends on your budget - I've read that elderly might appreciate stamps (if they're not into e-mail), maybe a small all-occasion package of greeting cards, calling cards, etc.
Some things for after - group photos, address books with addresses of all attendees so others might keep their records updated.

When our family gets together they like to play bingo. you can have people give 25 cents (or 50 cents) per game and the winner takes the pot.

Well old people love dominos, cards, Uno or may monopoly....these suits them as it is quite long and a bit interesting if they have peanuts and wine or beers together..and not to 4get their cigarrette....

how about charades ? BUT you will have to find some old t.v. & films for them to guess etc-- dads army -- steptoe & son -- morcombe & wise -- spring & port wine -- last of the summer wine. these would be ok. if it was upto date stuff they may not know the answer.
