A microscopic assessment is made of a fresh stool sample (within 24 hours). There may be more than one species of parasite present therefore the treatment regimen will vary depending on the species involved. There is no single "wormer" effective against all the different parasites. An examination for internal parasites in kittens should be done early since many of these parasites are transmitted from mother to offspring and thus have their effect on the very young. Early detection and treatment will enable the kitten to grow to its fullest potential while minimizing the health risks to humans. Repeat stool checks are recommended through the first year and then annually for all adult pets who have access to other animals or to the outdoor environment.

The only way to know without having a fecal test done is to see the worms; they may come out in the stool or sometimes the kitten may vomit up worms. I'd deworm him whether you see anything or not; most kittens that have been outside have at least some.

Watery stools could indicate Giardia, which is a protozoan. This can only be diagnosed by a vet and the medication is prescription only. If deworming the kitten doesn't improve things, have him seen. (He's going to need a checkup and shots anyway.)

I would at least TALK to a vet. You won't really know and this is probably not a very good place to get an answer anyway. Just to be on the safe side, talk to a vet, and then decide what to do. Just because the kitten was outside doesn't mean it has worms. There could be a large number of reasons. Giving it worm medicine without a diagnosis might be a very unwise thing to do.

If it's a stray kitten it's a sure bet that it has worms and over the counter drugs for this are a waste of money. Just go to the vet and buy some proper worm medicine that will work.
The diarrhea could be because you are feeding the kitten a different food that it is used to eating. Anytime you change an animals food they will have diarrhea for a few days until their bodies adapt to the change.
Watch for other signs of illness like goopy eyes, runny nose, vomitting and lethargy. These are signs of serious illnesses and your kitty will need to see a vet asap.

As gross as this sounds even vets will tell you to do this. I know because I rescue animals. You have to watch them go after they are done get them out before they cover it up and look for anything moving. The color and shape will tell you what kind of worms if it does have worms. There are over the counter de wormers but you need to know what kind first. If it does have worms you could look it up in the web or ask a vet for free what kind it is and then treat it. While treating it you still have to monitor its poop to see if the treatments working until its been worm free for about 2 weeks.

Any adopted stray needs to go the the vet... period.
You can pretty well count on worms in most cases and what you describe could be caused by any number of things. If you like the kitty and you want to keep it, a trip to the vet early on can help get and keep it healthy.
If you cannot afford it, check with your local humane shelter and animal rescue groups... one of them might help out with the vaccinations, spay or neuter and a general check up.

How do I know if my pet has worms?

Cats: A number of intestinal parasites will produce no visible symptoms in your cat at all – or at least not at first. Other parasites such as the tapeworm will usually be visible in your cat’s feces and in the hair or on the skin around his anus.

Other symptoms of worms in cats may include:

Vomiting
Persistent dry cough
Swollen abdomen
Anemia
Diarrhea
Persistent scratching around the anus
Weight loss
Eating a great deal without putting on weight
Breathing problems
Bad breath

It sounds like it could be a parasite. You should let a vet look at him. My kitten did the same thing and one time he pootied and there were little white worms in it. You really should have the little guy checked.
