A friend of mine has had a pain in her left side just under the ribcage for a few months now. It used to be a dull pain and now it's a sharp, stabbing pain and can be felt from her back too. What could it be. She needs to see a doc, I know but she won't.

It could be anything from gas trapped in the large colon right where it takes a sharp turn to cancer in that area. Another thing it could be is the spleen or the lungs (pneumonia/cancer) They "refer" pain to this area.

Like everyone before me said, she needs to see a health care provider for some very simple and not very expense testing. If she goes to the ER it will cost much more than going to a primary care doctor's office. NOT an urgent care clinic. Their cost is higher than doctor's offices usually.

Help her get there and good luck

She needs to see a doctor right away. Delaying and self-diagnosing is not going to help and if anything is to be done to prevent some unknown its best she be proactive and see her Dr. right away
