Both are correct.
One is generally asking about a person's health or psychological condition, with an expected answer of 'better now' or 'worse'.

two: Can be asking about physical symptoms or sensations (i.e. touch) or emotional feelings, with an expected answer, "Anger", 'love' or 'the water pipe under the dishwasher'.

The second one the doctor would ask when sticking the needle in your arm, the first one is what they ask when the pain killers kick in.
