When you register, you are registering to donate stem cells either through a traditional bone marrow donation (which others have described -- it's under anesthesia). Or what is called Peripheral Blood Stem Cell donation. The latter is becoming much more common. When you donate PBSC's, you take a medication called filgrastim which causes your body to circulate more stem cells into the blood. When you donate, they take blood out like a blood donation, spin it around in a centrifuge to get the stem cells, and then put it back. It takes a couple of hours and doesn't hurt any more than giving blood.

yea it does hurt a lot , we had a drive to save this little girl a few years back who needed a bone marrow transplant and many people donated their marrow , i dont think the chances of matching anyone elses marrow is very high though , they only used the marrow of a family member of hers because noone elses would work . but if you want to donate then good , it might be able to save someones life if they have the same blood type ,etc. as you .

You're under anesthesia while they do the procedure.

Marrow donors can expect to feel some soreness in their lower back for a few days or longer. Donors also have reported feeling tired and having some difficulty walking. Most donors are back to their usual routine in a few days. Some may take two to three weeks before they feel completely recovered.
