There should be rules, already in place, prohibiting a student from entering the teachers bathroom. There are no circumstances where teachers & students share the same bathroom. Check you guidelines. If you really do share the same bathroom, lock the door. If it was an honest mistake, then a simple "Excuse me, I'm so sorry" would be appropriate. If it wasn't an honest mistake, then detention comes to mind. Afterwards, do what you have to do the change the rules.

I don't know IF there are "standard procedures". However, if I were the student, I would apologize and leave. If I were the teacher, I would start locking the bathroom door.

My question would be why didn't that teacher lock the door? If a student did that, they should say excuse me or sorry and leave. The fault is with the teacher who didn't lock the door.
