It depends on what your talking about. Orgasms or breast or lips. Orgasms for most guys would like to imagine them being real but depending on if the woman wants to impress the guy I imagine most are fake. As for the cosmetics it would be nice to be real but alot are noticed at a sudden glance and are fake but they don't know till some research is done lol. All in all I guess you could say real is best if that was an option which most of the time its not

Ok...so serious answer? Real. I've felt and played with both, the fake ones are nice, just sort of...well...less respectful. I mean, if I'm in bed with a girl having sex, and she has fake breasts, I feel like I'm in bed with someone who wasn't happy with herself. I have less respect for that.
Also, I've heard women say they want them to be more "perky", up higher. Hellooooo...men don't like that! We want beautiful, naturally shaped breasts. Like the guy earlier said, they look like a Barbie doll. We like the softness of them, whatever size they may be.

If you are talking about an orgasm then the real thing. But if you are talking about breasts then again, real is better. They feel so much better. And more comforting

real, fake is so so shallow, and that's the biggest turn-off in a girl.
