Ok, so single phase to ground is easy. Phase to phase on a 120º phase shift system is V x square root of 3. What about three phase?

Looks like you are calculating voltage not power. If it is Delta wound then phase voltage = line voltage. If it is Wye wound then phase voltage = Sqrt 3 * line voltage.

kW = I*E*rt3*PF/1000
I = Amps
E=V
rt3=1.732
PF=Power Factor (generally .8 for inductive circuits)
