Does anyone know of any sites to down load authorization forms for a traveling child also permission to travel

How do you expect any person to answer this question? You give no information, not even the age of the child, what country the child is in and traveling to, what form of travel, whether the child is travelling alone, what medical conditions the child might have, where the child is to be expected to arrive, who is going to meet the child, etc. Then there is the question as to whether there are court orders regarding the child's custody and placement.
