Wow,a tough choice.I would first take the lady to the hospital and leave the other two some water.Than when the lady is in the hospital I would come back for my best friend and the girl of my dreams who would sit in my lap.

Well if the women of your dreams see's you help the old lady she want you even more, make my best friend wait with her and then come back and pick them up you can fit 3 people in a 2seater

I'd get my best friend in the passengers seat, get the old lady to sit on his laps, and the woman (or Man, whatever!) of my dreams to sit on mine.... and we have a speed-crazy drive to the hospital before two people die - the old woman - of whatever illness is killing her, or myself - of having such a hot woman on my laps! hehe
