I think that they very well could be real medical records. The people who wright the records are only human and can be prone to gramatical errors just as much as anyone else.

I have seen medical records that were similar so yes I believe it. Sometimes people don't read what they actually write and dont realise that what they wrote wasn't truely what they meant.
