Let it cool completely. (Maybe even chill it in the fridge.) Then wrap it.

Let it cool on the counter THEN wrap it in foil. If you put a warm sandwich in foil the steam from the heat will build up and make the sandwich soggy. But if its a cool dry sandwich it should be okay....it wont be warm though which would be the POINT of the foil....but foil wont keep it warm that long anyway...

Yes. Wrap it in paper towel or napkin and put it in paper bag.... Still get somewhat soggy but not as bad as foil or plastic.

Yeah..the cheese and butter have moisture, combined with the heat and no way for the steam to get out....waalah! you have a mushy cheese sandwich.

Yes. Butter and cheese are both solids suspended in liquid. If not kept refrigerated, they will break down and saturate your bread.

yea...the heat from the grilled cheese itself will create steam and well, steam the sandwich.
