or do you just go to the social security office and do a name change

Take your current social security card, marriage certificate, drivers license, and possibly birth certificate to the social security office and update your name. You should also change your drivers license ASAP. Once you get your new card DL card update all your other accounts (bank, credit cards, bills, etc) so you can have proper ID to prove its really you.

We were given our certificate the same day. I took that to the DMV and had my driver's license changed first, then went to the bank to have that changed and last went to the Social Security office (you can download the papers you need to fill out from their website; it makes it easier).

I received a marriage certificate in the mail a couple of weeks after my wedding that I have to bring to ss office.
