The optometrist has an eye acuity chart, with large letters at the top, graduating down to very small ones at the bottom. 20/20 vision means you can see the line you are supposed to be able to read, from a distance of 20 feet. In other words, normal vision. Some people have 20/10, which means they have to get to within ten feet of the target line before they can read it (without glasses). Others have 10/20, which means they can read the 10 foot target line from 20 feet away. That's not necessarily a wonderful thing, because it could mean they are far-sighted. They can see things at a distance but can't see clearly when things are close up.
There's a whole range on the chart, from 10 point to 100 point (the size of banner headlines on the newspaper).
It's just a quick guide as to whether you are myopic (near sighted) hyperopic (far sighted) or if you can read some letters in a line but not others, astigmatic (imperfect curvature of the eyeball. It means for example, that you might be able to see all the up and down lines in the letters, but not the curved lines.
With this information, the optometrist can more quickly hone in on your problem, and it's also a cheap and easy way to do mass screening.

20/20 vision means you can see something at 20 feet that a "normal" pair of eyes SHOULD see at 20 feet. If your vision is 20/200 then it means you can see something at 20 feet that "normal" vision could see at 200 feet if you have 20/400 then you have to be 20 feet away to see something that a "normal" vision would see at 400 feet.

If you put 20 apples on the table in front of you and you can see all 20 of them, then that's 20/20. If you can only see 12 of them, then that's 12/20. See how it is??
