There's no set time. You just have to communicate with whoever you are involved with and find out if they are interested in making it exclusive. Communication is key because once everything is out in the open the both of you can make the correct decisions and there won't be any room for confusion.

It varies. I've dated a guy for a month and then we became exclusive. I've also dated a guy for 3 months before becoming exclusive. I'm still with the guy that I made wait 3 months and we have been together almost 4 years now. Much longer than any other relationship I've been in.
Talk to the guy/girl to see how they feel about being exclusive.
