If it's done properly, it does not reverse itself. What usually happens in the vasectomy procedure is a small piece of the ducts from each testicle is removed and the duct is tied at both ends. If the duct is just stapled closed, then yeah, it possible to slip and open the duct. But, that's not normal. And, reversals are expensive delicate surgery.

During a vasectomy, the vas deferens are severed. Unfortunately, sperm do remain in the vas deferens closest to the ejaculatory ducts. This is why it is suggested for couples to use a method of birth control for up to 6 weeks or 10 ejaculations after the vasectomy.

There are cases out there of couples having children after vasectomies and tubal ligations. It is rare but it does happen. Nothing in this life is guaranteed!
