So many questions... so little space in which to answer.

You may find it useful to contact a few insurance agents and ask to interview them. I am certain that you will find several willing to answer all of your questions.

Most insurance companies have agreements with IMO/FMO, independent marketing organizations or field marketing organizations, depending on the insurance company. These organizations are responsible for training & supporting agents in all areas. These organizations work with brokers, agents, providing appointments with many companies as needed. Now, if you are captive or pursing this, then this changes a bit.
