I go by age, if they have been married before or if they are an older couple, they already have everything (though it will be a mess to get though...) to start their life. As far as younger couples, some may not even be out of their parents house, so cookware, kitchenware and bedclothes are always welcome (been there, done that :). There are also gift cards to where the couple is registered. Even $10 or $15 gift cards can help them get exactly what they want.
To me, it just seems better to "equip" then to "gift". Personalized gifts with wedding dates and names are neat, but there are only so many blankets, photo frames and Christmas ornaments a couple can use. I have stood in five weddings and have had my own, and I can understand the suspense of the couple when they do not get what they need to start off their life.
Regardless, whatever gift you give, hope that it is WELL received. Guests do not HAVE to give anything other then a card with their written best wishes.

well, i have been to a few wedding and have known quite a few that have gone to weddings. Technically, you are suppose to give cash inside a card. The cash is suppose to equivalent to what the bride and groom has paid for your plate at the wedding. Now most people give a gift because apparently its somthing that they need if they registered for it but if you are good friends and know them well. I suggest getting them a gift as well as a card with cash. Hope this helped! Good luck and have fun at the wedding!!

I tend to give cash to older couples, people who have previously been married, or couples who have lived together prior to the wedding. They have pretty much established the toaster thing.
If it's a young couple who may not have lived on their own, I would definitely get them a gift that could be used right away.

Give money. I got married 4 weeks ago and that's all that we really wanted. Everyone can use money, especially after spending so much on the wedding.

...gift registry.
Do what you think is best and fits your way of giving.
I can tell you that I appreciated all the gifts I received when I got married - cash and registry gifts both.
