Some friends said that this form is created with WORD, no, it is created with WORDPAD! so the point is : how to creat this kind of form with WORDPAD !!

In Microsoft word there are form elements that you can add to your word doc.

from the help file:

Create a printed form
1. Create a new document.
2. Design the form by sketching a layout first, or use an existing form as a guide.
3. Add the text or graphics you want. For example, enter the questions you want answered, and list the available choices.
• To insert a text box where users can enter their responses, click the document, and then click Text Form Field on the Forms toolbar.
•
To insert check boxes that list choices, such as Yes and No, click the document, and then click Check Box Form Field on the Forms too4. Save the form.
5. Print the form.
