Perversion is in the mind. There is no one act that is 'most perverted', since everything under the sun has been done and seen at one time or the other in history, HOWEVER, our society guages 'perversion' according to our societal mores and laws. Take it from there.

Social deviance is in the eye of the beholder. For me I'm thinking perversion is damage to oneself or others. To others it can be so varied depending upon the will of the recipient. To oneself I guess I'd have to go with an Australian aboriginal rite termed: "subincisizing". Not pretty.
