A gag gift is not ok. People don't like to get useless junk to clutter up their house. I say if you know this person well, then give them something from the heart that says they are special. If you are merely an acquaintance get them something useful, that can come in handy around the house. A gift certificate to someplace they enjoy would be nice too, not just any gift certificate, like McDonald's though.

One thing we've for 21st, 70th, or other milestone birthdays is put together a collection of small things--some general, some related to the birthday person's interests. For example, 60 chocolate kisses, 60 golf tees, 60 ball markers, 60 individually wrapped tic tacs, 60 pencils with their name on it, 60 bags of skittles, 60 pieces of "60" confetti. Each set is individually packaged, then it all goes in a huge box. Nothing needs to be really expensive, and you can get a bunch of people to go in on it if you want to get a little bit fancier stuff.

It depends on your relationship to the person, if you can joke with them about their ages go for it.

"Magic pills," a cane, fiber pills, prunes/prune juice, a commode, a catheter, a hearing aide, etc. are all good gag gifts for the aging group.
