Dry and/or greasy skin due to poor oil composition of the skin. Thick greasy oils clog pores, lack of oil leads to dryness and irritation. You need thin oils to moisturize while dissolving and clearing gunk in your poors. Try fish oil or seafood. 2 tsp fish oil a day or 4 servings of seafood a week. Stick it out for at least 2 months; it will take a long time to replace all your oil. Any effect after 1-2 days is temporary or random; so even if it makes you break out a little at first, you haven't given it a full try yet.
In the short term you can wash and moisturize well, but that will only go so far. Plus excessive washing can be drying and excessive moisturizing can be clogging. Use a small amount of a light moisturizer, made with oil not jelly or grease. Often that means soybean oil or mineral oil. Mineral oil means mined from the ground. So soybean oil is usually better, though mineral oil won't cause too much harm. Clean with soap and water, not a harsh acne cleanser. Even then they only work so well. So you really need the seafood.
Antibiotics aren't really good for bacteria long term, they'll come back in force after. Short term they may help. After you get off them find some kefir with acidophilus listed first or 2nd to replace the friendly bacteria they destroyed. Studies show less illness when you have these bacteria, even outside the stomach in places such as the lungs and elsewhere. You want friendly bacteria to fill the void when the antibiotics stop, not harmful ones.
In the short term you might also try 100,000+ iu retinol vitamin A (a megadose, and too much for normal use) or one of the acne drugs that is similar to retinol vitamin A. It's some minor harm to your organs, but it helps against bacteria on your skin. At least it doesn't have the other long term drawbacks to your skin that antibiotics and many scrubs do.
If you would like to learn how to treat your acne permanently and regain your health and wellbeing, without drugs, without typical acne treatments, and without any side effects, then this will be the most important letter you will ever read.

try proactive. it's been a success for many people. But you have to stick to it, everyday, every night. If for some reason you don't do that, you have to get a gentle soap for your skin, something without fragrance and dye, try purpose twice a day. start taking vitamin A (10,000 I.U.) and zinc, (50 milligrams) once a day, everyday. A simple multi-vitamin can help also. It can take up to 6 months with vitamins to see results. You have to start eating healthier. My diet definitely affected my breakouts, even though dermatologists beg to differ. Start eating more applesauce, carrots, grapes especially, and less fried foods, less salad dressings, less potato chips and dips and less carbonated drinks, although if you pay attention to your diet, you'll be able to tell what foods cause you to break out. Get a good scrub/exfolliant to use once a week. Will help oily skin especially, and it gets rid of dead skin cells faster. If you break out mostly on your forehead, you have to start washing your hair more often than you are now Get a sunscreen. SPF15 Oil of Olay for sensitive or oily skin is good, the generic equate is just as good and alot cheaper. P.S. If Clearasil isn't working for you, try something the active ingredient salicylic-acid.

Try Neutrogena Pore Refining Cleanser.
And 100% pure Tea Tree oil as a spot treatment for pimples.
Only use tepid water when you wash.
Use gentle pressure, as gentle as when you shave your legs.
Do not pick, you spread bacteria when you do that.
Also until you are all healed up:
Keep your hair off your face. Ponytail or scarf headbands.
Change your pillowcase everyday
Use a fresh towel to dry your face, pat don't rub.
Don't allow hair products like hairspray to get on your face.
No prolonged kissing (you shouldn't at your age anyway)
Don't let you pet get in your face.
Avoid anything you are allergic to.
Get a small amount (15 mins) of sun each day
Avoid makeup with talc in it, including many powders and blushes.
Avoid sunscreen with PABA
Avoid aspirin/ Ibuprofen and other anti-inflammatory drugs (they interfere with your body’s histamines response which allows white blood cells to get in there and fix the tiny infections that are acne.) It is ok if you really need it, but if you want to really do everything you can...
Good Luck

You've got to at least try what I'm using! I use the Clear Skin Regimen at http://www.acne.org. It's a non-profit (so it's free), the guy who created used to suffer from pimples himself. The system works wonders on my face, and its helped countless other people. The best part is that you don't have to order or buy any expensive treatments, everything you need for the Regimen can be found at your local drugstore. Find out more at the website.
In general, for clear skin it helps if you: don't touch/pick at your skin, drink lots of water, wash your hair frequently, change your pillowcases, exercise, wear little make-up (or if needed, hypo-allergenic makeup), and minimize stress.
Another tip is to look up online consumer reviews of acne products before you buy them, using sites like epinions.com or drugstore.com. A great website I use to get cosmetic reviews is makeupalley.com (A mostly women's site). Hope this helps!

Yes over washing is not good for your skin. Proactiv works great, but it is expensive in the long run. Try only using something at night before going to bed and wash your face with cold water, only, in the morning.
Drinking or eating crap isn't the problem, but it will affect the quality of your skin, not your acne.

Maybe your washing to much and with to harsh of a cleanser. Thy using a mild cleanser and back off of washing twice a day.
My 17 year old daughter was doing the same thing you are doing and she had the same problem. She did what I said and the break outs have calmed down.

Sometimes it happens because you are growing up and your hormones are working. You really can't help it. There are medications to take by mouth but some of them are dangerous. Keep washing and try not to touch your face too much or let your skin dry out.

Stop drinking soda and drink tons of water. Since I've had my braces I didn't drink soda and my face is pretty clear. But along with drinking water I use Avon clearskin. I think it works better and it's cheaper to. Lay back on fast food too eat more nutrious meals. But the water thing will help alot too. Hope that helps.
