Clean the whole interior thoroughly doors, headliner, carpets, seats, etc. with soapy water or most any type of cleaner for upholstery. I use "Tuff Stuff", works well and leaves a nice clean scent. This can be purchased at Wal Mart in the automotive section. Keep the windows cracked open to air it out. If it is real bad this may have to be done more then once, but it will get better in time.

You are going to have to run the air with the windows down to get the build up smell out of the vents. Then you can do a couple diffrent things. Clean the upholstry with a carpet cleaner, use air freshners or pick your favorite fabric softner dryer sheets and stick them under your seats and in any nook or cranny you can find.

An automotive detail shop really is the best answer. Used car dealers have the same problems you describe. For them, it's worth the expense. But, since it is very expensive, you might try some of the less expensive suggestions on the list. They may work well enough to satisfy you. You might consider renting an upholstry shampooer.

Use Nature's Miracle on the seats and headliner (if the seats are not leather). Also, thoroughly wash the carpet. Nature's Miracle is typically used to get pet piss and poo out of carpet, but works with other odors too. I bet you could find it at a super market. If not, they'll have it at a pet store.

I wouldn't recommend using air fresheners unless you like that smell. I hate air fresheners. I think they smell like sh1t.

Try vacuuming the truck out and put a couple air fresheners under the seats and on the review mirror.

i'd advise moist-vaccing the upholstry and punctiliously sparkling the demanding surfaces as nicotine and tar create a movie on those surfaces. you gained't thoroughly get rid of the smoke scent, yet doing both those issues will cut back it a great deal.

believe it or not I've always heard Massengil - the feminine hygeine product - gets out smell. A co-worker's house burned down and that's how she salvaged her clothes. Maybe use it as a wash for the floormats and interior?

Wash the interior. Wash all the seat covers, roof linings etc. Also buy a ton of air fresheners ;)

You could scrub all the hard surfaces with a disinfectant, but you'll probably never get the reek out of the upholstery/seats.
