Mathematics is a way of describing the world and it's functionality. It's like english, but with numbers and precision. Physics, as well as other sciences, is the application of mathematics to the physical world. Physics uses math to describe how things work.

Math is not a nature science, but derived from parts of philosophy, for example the basic algebra of logic's (Things like: "One true statement connected with another true statement by 'and' gives a new true statement). A mathematical proof is to show that a theorem is consistent this (and other) basic assumptions by showing that it's deducible by applying these rules which are axiomatic and not provable by themselves.
Physics is a nature science, and has the experiment to "ultimately" test a hypothesis. So although physicists use the same language as mathematicians, physicists have a additional test for their work: Nature itself, as physics is interested in real systems, not in all possible systems.

The key difference as I see it is the difference between deductive reasoning and inductive reasoning.
Mathematics is a discipline based on deductive reasoning - starting with a set of known principles, postulates, etc., mathematics is a tool that allows one to calculate, predict and extrapolate from a known set of quantities (your input) to a desired set of quantities (your output).
Physics uses inductive reasoning. Unlike math where the starting point (the rules, postulates, principles, etc.) is known, in physics the object is to discover those rules (how the universe workd) based on measurements and observations. Then, in theory, once those underlying rules of how the universe works are discovered, engineering or applied physics takes over and the rules are applied to a problem.
Physics uses math as a tool in two ways: first, to verify that the theories are correct by comparing the calculated quantity to the experimental observation; and second, to extrapolate those principles to new situations.

When you get into the really high level math and physics they seem to mesh into each other. But really the physicists often use the math scientists to assist them in making their equations, or that is the idea anyway. The Math scientists are better at equation "shenanigans" so to speak, while the physicists are better at hypothesizing measuring and designing experiments to determine theoretical values. Clear as mud?
