The manager is the one who has official control of his team's lineup card, and is the only one who is allowed to change it.

However, if the manager is ejected in the course of the game, his substitute (often a pitching coach) becomes the keeper of the lineup card and then can make substitutions.

I believe that the manager is the only one who can take a pitcher out of the game. But if the manager is thrown out, the acting manager, usually either the bench coach or pitching coach can change pitchers. If the pitcher is thrown out of the game, then the manager chooses a reliever to put in.

Manager can of course, if he is ejected then whoever assumes his duties as manager can, so its still the manager officially. Other than the pitcher getting ejected by an Ump. Oh also the opposing players can nail him with a line drive and take him out... Jus Kiddn

Techincally, the pitcher can, the pitching coach can, or the manager. However, the manager is the only one who can offically change the lineup card for the ump, so he has the last word.

Nope. Especially if he's been kicked out of the game. They don't have to leave the same pitcher in the game the rest of time!
