PASSPORT REQUIREMENTS

If you have never been issued a Passport & are a natural born US citizen, you will need:

*Form DS-11 Passport Application
*Certified copy of your birth certificate
*Two passport photos
*Photo ID (preferably a drivers license)

You will generally receive your passport in 4-6 weeks. If you require expedited service there is an additional fee. With expedited service your application is processed within 3 business days of when it is received by the issuing office (this does not include mailing time). Passports will be sent by regular mail unless other arrangements for mailing are provided by the customer.

Most county courthouses have an office to pick up forms. Some post offices and other businesses offer services as well.

Certified copies of birth certificates must be obtained from the Vital Records office of the state in which you were born.

in the US - you will need a birth certificate with a raised seal on it. you can get this from the county courthouse in the county you were born in. you will also need some other form of photo ID, like a drivers license.
