They are not mutually exclusive except for health care. For a human to survive for any length of time, water, food, shelter is required. Clothing is actually a form of shelter to protect our fur-less bodies from the elements. Health care--unnecessary if you have the others in perfect proportions.

Water. Healthcare, shelter and clothing you can make do without for awhile, food is necessary but not as much as water. Since our bodies are mostly water, one cannot live very long without it.

You can go longer without food, shelter or clothing than you can without healthcare or water. I'll say water. Lots of healthcare is administered by quacks.

You need only your third paragraph. BTW, they started in 1965. It's called Medicare, and it was passed on a promise that by 1990 it would only cost $9B (actual cost $67B). Once you control 15-20% of the economy, the rest gets easier.
