Consult with an attorney AND your doctor first! Anything on that form can be used against you to try to reduce the payout for your injuries -- or even possibly to avoid any payment at all if they think that a pre-existing medical condition contributed to the accident and you did not disclose it previously!

The ONLY reason that insurance companies use these forms is to REDUCE their liability! That ultimately means less $$$ for you.

Insurance company's are infamous for trying to get you to settle as quickly as possible, so fill out the form but if you're still in need of medical help, make sure you include this on the form and don't sign anything until you're completely well!

Well, they're going to use it to see what kind of settlement they want to offer you. If you DON'T fill it out, they're not going to offer you any $$. If you DO, you can't change your mind later.

If you have injuries, you might want to consult with an attorney (or at LEAST your agent) before you fill this out.

stop!!!!!!!!!!! fill nothing out!!!!!!!!!!!!!!!!
see a friggin lawyer, you can bring the form and get a free consult, ask the lawyer then if it is ok and if you may have to sue then let him handle it all, never sign anything like that or anything else that might get you jammed up or limit your options in the future.

they will deny your claim and they will us it to find out your past medical history so they can reduce your payout.
