That depends on where you live. Usually an unexplained death is autopsied, in some counties, all deaths out of medical setting are autopsied. If a family requests one it can be done at the family's expense. An expected or explainable death isn't usually autopsied. There must be more to the question than you have posted.

It depends; because the autopsy and all the lab tests cost money, it is usually only performed when there is some question as to cause of death. If a little old lady of like 90 falls on the street corner clutching her chest, chances are it's a heart attack, and no one's going to question it. Same thing for a shotgun blast to the head -- the cause of death is pretty straightforward, so there's no need to go rooting around in the guy's spleen, though they might do bloodwork to see if drugs or alcohol were involved.

It depends on the area and the wealth of the area. For instances, some more populated areas, like New York, might perform autopsies regulary. But some small towns like Po-Dunk, Idaho, might never perform autopsies. It all depends on area, money, and the circumstances surrounding the death.

Autopsies are usually done at the request of the family unless the deceased was a minor/child. I'm definitely sure they don't do an autopsy on everyone that dies. I'm also confident that they have to do it on the kids if they somehow die.

It seems like autopsies are performed more and more these days, before they were only used for certain deaths, such as murder, sucide, and I think if someone dies and there is so sign of how. I think that its more of a family decision now to see how the person really died.
