The name tag on my dogs collar is to the point you can't read it anymore, are there any good sites to order a new name tag? I keep forgetting to swing by the vets office to get a new order form so I would like to order one of line.

if you have a PetCo, PetSmart, or a big store near you, they have machines that will engrave them while you wait...
otherwise...drs foster and smith is a great website/catalog for pet supplies

You can make one for about $5 or $6 at Petsmart. Just go to the register and pay for the tag. The cashier gives you tokens for the machine. You can choose the size, shape and color of the tag and then personalize it with (I think) up to 4 lines of text. I spent a about a dollar or two more for a brass tag about 3 years ago and it still looks great.

You may also want to look into a Quiet Spot tag sliencer. It is a small pouch that goes over the tags and keeps them from banging together, thus reducing wear on the tags. You can get them at some pet stores or pet catalogs or from thier website: http://itzadog.com/quietspot.cfm

try the wal - mart in Holland, Michigan. they have nice looking name tags for up to 5.00 which are tie - die.
