There is a form you have to fill out at your local Social Security office, requesting a duplicate. I believe that due to identity theft you have to do it in person, and show ID. They will give you a receipt with your SSN on it and mail your card in about 2 weeks.

Go onto the social security website - there is a form to complete. You may be able to mail in the form with the required info to get a new card issued to you. If not, take your drivers license and your birth certificate to a local office (there is one within driving distance of every city). The form comes with instructions, and will tell you what is needed.

You'll need 2 forms of id( the approved list is online or you can listen on their phone automated system) and fill out a short application to be reissued your cards. The form is the same one when you apply for the first time. You can download it from their website, or call the automated system and have it mailed to you. Complete ss5, have your id's, head to your local ss office, grab a number (early in the am is best) and a rep will take the docs and process. If you need a letter for interim before they mail to you. They will give you it on the spot.

You can go to your local social security office and apply for your card. Or, go to the website and download the social security application form and print it out. Fill it out and take to the office. Take all pertinent information so the process won't be so bad. You might also want to take something to read, you might have a wait. Good luck!

Go to ssa.gov, download the form to request a new card, locate the address for the ssa office in your area. Mail it in with the proof required, usually your original drivers lisence or birth certificate...yes the ORIGINAL, but they mail it back within a few days with a letter saying they got your application...then you will get your card within a week. The best part is that it is free minus the stamp.
