He's not happy in his relationship. Should I tell him how I truly feel or let nature take it's course?

I would let nature take its course. The reason why is you don't want to be one of the reasons he left his relationship. Now when that relationship ends for good, then let him know how you truly feel and then you can proceed without any guilt. You go girl. Be Safe and Be Happy.
