I'm finding that most guys are flattered if a girl asks them out, so don't be frightened. Ask him to do something very casual and make it sound casual - like - hey, we should get together for a drink/coffee/soda sometime. That's a pretty harmless invitation. If you're high school age, ask him to an after-school function, like a football or basketball game. Say "I'm thinking of going to the basketball game Friday night" wait for his reply and if he sounds remotely interested ask "you should come with me."
Good luck & don't be afraid. Remember, 100 years from now, no one will know or care if you asked this guy out.

Say "Hi, I've been seeing you around for quite a while now and I just have to ask you, would you go out on a date with me?"

Just say, "Hey _____, I think your cute and I was wondering if you would go out with me."
However, I have asked a guy out before and he said no, just wait to see if he will ever make a move.

How about saying I am going to the movies this weekend or the park or etc and ask him if he would like to join you

just say "are you doing anything this weekend? cuz im going to ((insert place here)) n i was wondering if u'd like to join me?"
but that's only a suggestion. there are many other ways to ask him out. good luck!

Start a small conversation and then ask him if he likes you if he doesnt already know and if he says yeah then ask him out.
