Spit, sometimes there are little pieces of "stuff" that has been removed during the procedure that can cause you to choke if you swallow. Don't worry about bacteria as suggested in another answer, they're already all through your swallowing system.

Spit, otherwise you might become nauseous...especially after a flouride rinse. It contains something to make you gag in case you swallow it, per my hygeinist.

Of course you have to spit it out, it contains all the bacteria that has been dislodge during the dental procedure.
