Make a trip to the city-county building. You need to apply for a marriage license. Then contact the Social Security offices for a new social security card with your married name.

Changing your name is a pain in the butt....believe me, I'm in the process of doing it. You have to get a new social security card...make sure to take your marriage license with you. Once you get that, you can apply for a new license, and contact your bank, credit cards, utility companies, etc. to change your name.

If you are going to take his last name, go to the social security office and get a form to fill out and send in. This will change it in the social security office and you will have to go get your licenses changed, and let everyone else, your employer and others of the name change.
