It seems she is still new on the game, don't push it all, use a lot of K-Y, try with middle finger first, then use 2 and finally the tool.
She will get used to it, baby comes out of it and your tool is nowhere near it

You have to be gentle with her,over a certain period of time.Build her up gradually over weeks,months if needs be,until finally she can take it all.
