I might also need 1992, thru 95. I'm looking for an urgent care office that was on US 19 near Publix. They have some records that we need.

This is a tough problem and librarians are famous for smoothing out the wrinkles and getting you what you need. Meanwhile, see if the place below is the one you need (the name may have changed since the 1990s).

IF the office is still there, you can try the current year's phone book on line. If the office is not, no year will help. What is the name of the medical facility? It should be names after a plan like Blue Shield... You can try the County Hospital records department as well. Good Luck

call the local medical society they have records of that stuff, and if the clinic is closed then they will be able to tell you what they did with the records.
