They associate sexual pleasure with the object of the fetish. Sometimes it is just by chance exposure and other times it is the result of persistent fantasies. As the person experiences sexual pleasure along with the fetish (sexualization) they come to prefer or require the object in order to gain sexual satisfaction. This theory is used in therapy: a pedophile is asked to masturbate while viewing an adult until the biological process (and resulting pleasure) is associated with the image of an adult. The goal is to train the sexual preferences of the pedophile away from children and toward adults.

I would say by seeing or doing one thing that they find pleasure in, becoming addicted as a result of being exposed to it so much, and eventually only pleasuring in that one thing due to their high exposure to it and pleasure they have gained from it in the past. But that's just me.
