You can backup your important files into a cd, reformat your hard disk, reinstall the OS and the other programs and then manually copy your backup files.

Depends on if it is a virus or spyware. Some spyware or adware is so malicious that it will not go quietly. I use a combination of a couple of things to remove threats like that.
For software, I use Norton Anti-Virus (not that internet security crap that slows your machine down), Spybot, and AdAware. This combination has been the most effective in removing most threats. Each gets rid of somethings that the other wont. (NOTE: There is NOT a software program out there that gets rid of them all. Viruses, adware, and spyware change so frequently that no single app can keep up with them all.) Once I have ran all of those I begin a brute force attack on all startup applications, either directly through the registry (HKEY_LOCAL_MACHINE\Software\Microsoft\Windows\CurrentVersion\Run & HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Run) or, more safely, with msconfig. I prefer the first option.
After that I start removing folders from Program Files I don't recognize. If you are unsure, do a google search before you delete it. Then I restart clean. After that, I run windoctor from the Norton Utilites package. This cleans up the mess in the registry. This strategy cleans up 99% of the systems I have worked on, and can usually be done within an hour or two, depending on the level of infection. If none of that works, take the computer in to a qaulified technician and they will be able to do the above tasks without a problem. If they can't, take it somewhere else. It just means they are not qualified enough to do it.

Worse case, you can send me an IM on yahoo chat and I can connect to your computer and do it for you. I'm nice like that. :)

As the Sallymarlyn said, that is the most safy way to remove virus and trojjan. But I suggest you to disable Start items in your reg file under Windows Safe Model( even to remove those virus). Some of these unwantedware can scan your system area and modify it. You can remove them under Windows Normal Model, but one moment later they may appear in it again. So please do that under Safe Model. Just press F8 as you pc starts and before you see the system menu. choice Safe Model and press Enter.

Good Luck!
