We eat out a LOT -- two to five times a week, for different reasons:
1 - Convenience. With three kids (two teens & a 9YO) with varying schedules and activities, there are some days (Tuesdays -- voice & piano lessons for the younger ones) when it's just one less thing to worry about fitting into the schedule. Sometimes we grab a quick bite someplace, others we'll pick up takeout, but this is usually inexpensive and fast.
2 - Adult-bonding time. During the school year, my wife and I make time for lunch dates during the week while the kids are in class. They're like little islands of time apart from running the household and all the back-and-forth that goes on with kids and work and everything else, and we treasure them. These are a step up, budget-wise, from fast food Tuesdays but not break-the-bank expensive. (Budgeting tips to follow.)
3 - Celebrations. Our anniversary was last month and we had several nights out to celebrate it. Only one was actually on the date, and it wasn't even the fanciest/most expensive, but it was at a place where we feel special and comfortable. We also celebrate with the kids on their special days -- birthdays, school milestones (performances, recitals, etc.), and the like. Some of these are expensive enough that we do them infrequently, other times we just turn anything into a celebration, like a trip to Baskin & Robbins or Coldstone Creamery -- won't break the bank, but the kids love it.
Budgeting tips: I actually deduct a weekly scheduled amount for Dining from our balance in Quicken, then every time we eat out I adjust the amount remaining to make sure we don't overspend. This also helps us save up for a big event, like an anniversary, because we can economize for a week or two and make sure we can have fun on the big night without having to use a charge card. Deducting the amount in advance (I set up a scheduled transaction every Monday, for the week) lets me see how much money we'll have left, but it shows me the balance before we've actually SPENT the money and found out we're in trouble -- that's the key to doing this successfully.

Hardly ever, maybe once every few months or more. My wife is an excellent cook so there's not benefit to going to a restaurant, except to do something different or for her to not cook for awhile. When out and about and getting hungry, I usually wait until I get home to eat - even leftovers - because it's better than what I could get in any restaurant. And no, she is not pressured into cooking. She enjoys it. I enjoy the results.

We eat out when out of town, but for the most part my girlfriend lets me do the cooking. For a meat and potatoes girl she lets me cook all the pastas I want.
