it depends on the team-
I know that the ITALIAN National team's captain is always the player with most number of caps (games played, or chosen to play with the national team).
some teams choose their captain by the players voting for one, and others just have the coach/team president choose the captain.

In different places soccer team captains are chosen in different ways. In some cases, the whole team votes, on who should be named captain. In other places the team coaches and managers choose the captain of the soccer team. It all depends on where you are in the world.
