Horse shoes are used to protect the foot, as another poster mentioned. Horse's feet wear and break off at different rates. If a horse is used often on abrasive or hard footing, a shoe will help protect it.
Horseshoes can also be used to help with certain medical conditions, offering more support while the horse's foot heals. There's a condition called Navicular disease, where the bone essentially disintegrates, Bar shoes (circular ones, instead of 'U' shaped) stabilize the foot and inhibit spreading. Another condition, called laminitis (where the interior of the hoof separates from the wall) makes the foot especially fragile. In extreme cases, a supporting bone, called the coffin bone can rotate downward. A 'Heart Bar' shoe offers support like a bar shoe does, with a small plate covering the center of the foot (the frog) and hopefully support the foot as it heals.

The horses give them more support in their joints and keeps them from getting stone bruises on the bottom of their feet. They don't feel any pain when they get them put on either as long as the farrier doesn't hit their white line, which if he does this it will cause an abases in the foot. It keeps their feet healthier and rounder so they don't trip as much. The down side is your have to have it done ever 4-8 weeks depending on the horse because their toes will not naturally wear down like if they didn't have them, and will actually grow over the shoe.

Horses need shoes to protect their hoofs almost for the same reason you wear shoes. Horses are domestic and their hoofs don't get the wear and tear that wild horses get so domestic horses need to have their hoofs trimmed and most (not all) need to wear horseshoes.

Horses weren't designed to walk or run on hard surfaces, like roads. The shoes save their hooves.

A horses hoof is like the cuticle on your nails.
It cannot withstand the rigors of streets, trails, or pathways
without a little help. Basically it is protection. Not unlike
you and I wearing shoes.

It protects their hooves. They really need them if they are on concrete a lot.

They nail them into their hooves. Don't worry they can't feel anything though.
